CF Recreation Grants
The CFLF intention in approving a Recreation Grant request is to encourage and support access to activities that challenge the body and the lungs physically, while also providing mental, social and emotional benefit to people living with cystic fibrosis.
Applying for an Individual Grant Step-by-Step
1. Your information
Complete the online application and complete the household, personal, emergency and consent information portion of the application. Applicants under the age of 18 will also need a parent or guardian’s permission for HIPPA consent to communicate with CF care providers if needed.
You can also download a paper application (download links below) if you prefer to submit through the mail.
2. Your activity request
Provide the information of what activity you are requesting support for, and the details of where payment would be provided to.
3. Your photo
Upload a picture of you being active! Please provide a photo of yourself individually (not a group photo or headshot) doing the activity you are requesting support for, or another activity if this request is for something you have not done before. High resolution pictures are best!
4. Your letter of support (optional)
If you would like, provide a letter of reference from a health care provider, teacher, friend or whomever can speak to why you are most deserving of this grant. This is optional, and will count as extra-credit on the evaluation of your application.
5. Your story
In either a written essay (3-5 paragraphs) or a recorded video (2-3 minutes), share your compelling story about why you want and need this grant and how it will affect your life with cystic fibrosis.
6. Your agreement
Read through and initial each section of this agreement to use this grant to the best of your ability and share updates with the CFLF about your personal progress in using this grant.
7. Your CF Team
Provide your CF center and doctor’s name and the best contact person’s information for confirmation of your diagnosis.
8. You’re Done!!

Things You Need to Know
Recreation Grant requests may be submitted at any time. Individual and Peer Support grant applications will be evaluated with all submissions received by the cycle due date.
Individual Recreation Grant requests may not exceed $500.
Peer Support Grant requests may request a second $500 for a non-CF person to assist with the activity.
Loretta Morris Fund Grant requests support swimming, horseback riding, dance and golf. These grant requests are reviewed upon receipt and do not have a deadline. Requests for other activities should apply for an Individual or Peer Support Grant.
Canadian Grant requests are ongoing and do not have a set deadline.
Applications requiring additional information will have two weeks from the time of request to provide what is needed.
Applicants may only submit one type of application at a time.
The annual financial information for the household (including ALL members in the household) that we ask for helps us to evaluate financial need. Leaving this field blank will have a negative impact on the review of your application. If you want to explain special circumstances, please do so in your letter/video part of the application.
Only one Recreation Grant can be awarded per recipient per year.

A Letter of Reference may be included with the application, but it is not mandatory. If one is included, it should be from someone who is familiar with the applicant and with CF. Examples include, CF doctor, nurse, dietician, respiratory therapist, pulmonologist, or social worker.

Yes, we are currently accepting applications for the both cycles. For people who choose to apply for traditional activities (i.e. gym memberships, yoga classes, swimming lessons, summer camps, dance classes, etc.) we ask that the COVID-19 liability waiver be signed in the application. During this time, as a way to support those with CF who are not comfortable leaving their homes for recreation activites, we are opening up the program to include the purchase of exercise equipment. This option does not include additional funding for a Peer Support person. Those interested in applying for a grant for exercise equipment should fill out the Individual Grant application.
Yes, as long as the total of ALL activities is $500 or less you may apply for multiple activities. Requests made that are over $500 (for an individual grant) will only be funded up to the $500 maximum.
Eligibility is based on being at or below 400% of Federal Poverty Level for individuals with CF or families with one child with CF. Families with 2 or more children with CF are considered eligible at or below 500% of the Federal Poverty Level. To calculate your eligibility, find your family size HERE and mulitiply the income level by 4 (or mulitply by 5 if there is more than one person in your household with CF). If your household income is at or below that number, you ARE eligible for a CFLF Recreation Grant.
Applicants will be emailed with either an approval or denial approximately one month after the deadline. Based on the number of applications received it could be a week or two later. If you have not heard a decision about your grant request by 5/15 (or 10/15 for the second grant cycle) you may email Program Coordinator Tiffany McDaniel at Tiffany@CFLF.org
We prefer the photo to be of the activity the person is applying for. However, if one is unavailable you may submit a photo of yourself doing a different activity. Please do not send photos of just head shots, or non-active poses. All photos must be of ONLY the individual, due to privacy issues.
No, unfortunately requests made for reimbursement will not be considered.
Yes, if your child is unable or too young and does not understand the questions, their parent or guardian may help in filling out the applications on their behalf, from the parent/guardians perspective. It is preferred that as much of their own words are used in the application, so using the questions to ‘interview’ them is often a great option.
The only fundraising events or participation that the CFLF Recreation Grants would be eligible to support would be for CFLF.
Typically grants are only for activity costs (i.e., gym memberships, summer or sports camps, exercise classes, swim lessons, martial arts, etc.), but during the COVID pandemic we have temporarily allowed for purchase of in-home exercise equipment until further notice.

Grant recipients may re-apply every year for the same or a different activity. Individuals are eligible to be awarded one grant per 12-months which is determined by the date of approval. For example if someone is approved on June 1, 2017, then they will be eligible to be approved again on June 1, 2018.
The application should come to us fully complete, which means all pieces of the application (each page filled out and a photo) should be submitted together. Due to high volume of applications we can not guarantee applications that are sent in separately will be saved and kept together.
Have your doctor put a note on the page where we ask for PFT scores that the child is too young for lung function tests.
